Astfel, proiectul a generat urm\u0103toarele rezultate:<\/p><ul><li>un nou algoritm de postprocesare imagistic\u0103 \u0219i caracterizare a inflama\u021biei din \u021besutul epicardic bazat pe inteligen\u021b\u0103 artificial\u0103<\/li><li>un nou algoritm pentru evaluarea pl\u0103cilor ateromatoase vulnerabile bazat pe inteligen\u021b\u0103 artificial\u0103<\/li><li>un nou instrument de predic\u021bie pentru prezicerea riscului de evenimente cardiovasculare majore, bazat pe analiza inflama\u021biei de la nivelul \u021besutului epicardic prin inteligen\u021b\u0103 artificial\u0103<\/li><li>validarea predic\u021biei prin studiul de biomarkeri<\/li><\/ul><p>\u00a0<\/p><p>Principalul rezultat al proiectului, cu un puternic impact asupra serviciilor medicale de \u00eenalt\u0103 performan\u021b\u0103, \u00eel constituie introducerea, testarea \u0219i validarea unei noi metodologii de evaluare a riscului cardiovascular, bazate pe imagistic\u0103 neinvaziv\u0103 \u0219i care integreaz\u0103 inflama\u021bia coronarian\u0103 \u00een algoritmul de predic\u021bie. Superioritatea acesteia fa\u021b\u0103 de celelalte metodologii utilizate existente \u00een prezent este dat\u0103 \u00een primul r\u00e2nd de integrarea \u00een noua solu\u021bie rezultat\u0103 din proiect a inflama\u021biei de la nivelul \u021besutului epicardic, cu potential crescut de destabilizare a pl\u0103cilor ateromatoase coronariene, dar \u0219i de algoritmul bazat pe inteligen\u021b\u0103 artificial\u0103 de determinare a fenotipului CT al pl\u0103cilor coronariene.<\/p><p><!-- [if gte mso 9]&gt;--><\/p><p><!-- [if gte mso 9]&gt;--><\/p><p>\u00a0<\/p><p><strong>\u021aesutul adipos epicardic, inflama\u021bia local\u0103 \u0219i vulnerabilitatea pl\u0103cilor ateromatoase-studiu imagistic computa\u021bional bazat pe inteligen\u021b\u0103 artificial\u0103 \u2013 IntelFAT<\/strong><\/p><p><strong>\u00a0<\/strong><\/p><p><strong><em>UEFISCDI PCE 206\/2021<\/em><\/strong><\/p><h2><strong>REZUMAT EXECUTIV<\/strong><\/h2><p><strong><em>Obiectivul general<\/em><\/strong> al proiectului IntelFat a constat \u00een cercetarea rolului inflama\u021biei mediate de \u021besutul adipos epicardic \u00een cadrul procesului complex de vulnerabilizare a pl\u0103cii ateromatoase. Cercetarea a avut la baz\u0103 utilizarea unei solu\u021bii inovative pentru cuantificarea inflama\u021biei locale de la nivelul \u021besutului adipos epicardic din jurul arterelor coronare, bazat\u0103 pe post-procesarea imaginilor ob\u021binute prin tehnici imagistice avansate de Angio CT coronarian. Pentru realizarea acestui obiectiv, s-au efectuat:<\/p><ul><li>extragerea datelor din banca de date imagistic\u0103 avut\u0103 la dispozi\u021bia proiectului<\/li><li>dezvoltarea unor algoritmi de selec\u021bie \u0219i extragere a caracteristicilor imagistice prin AI<\/li><li>dezvoltarea de algoritmi de inteligen\u021b\u0103 artificial\u0103 pentru evaluarea inflama\u021biei din \u021besutul adipos<\/li><li>studiu de biomarkeri \u00een corela\u021bie cu caracteristicile CT de vulnerabilitate a pl\u0103cii coronariene la 158 pacien\u021bi cu inflama\u021bie accentuat\u0103 la nivelul \u021besutului adipos<\/li><li>testarea \u0219i validarea algoritmilor de predic\u021bie a riscului de sindrom coronarian acut<\/li><\/ul><p>\u00cen total, tehnologia de detectare a inflama\u021biei \u021besutului adipos a fost testat\u0103 la un num\u0103r de 760 artere coronare ale 261 de pacien\u021bi, a c\u0103ror imagini au fost extrase din baza de date \u0219i procesate, iar rezultatele au ar\u0103tat rolul inflama\u021biei de la nivelul \u021besutului epicardic \u00een vulnerabilizarea pl\u0103cilor ateromatoase. Astfel, am demonstrat c\u0103 o cre\u0219tere a inflama\u021biei poate identifica sediul viitoarei rupturi a pl\u0103cii ateromatoase <em>(Benedek T et al, Congresul EuroPCR, Paris, mai 2023). <\/em>La pacien\u021bii post COVID s-a ar\u0103tat o vulnerabilizare mai crescut\u0103 \u0219i o inflama\u021bie mai accentuat\u0103 la nivelul coronarei drepte <em>(Benedek T et al, Congresul Colegiului American de Cardiologie, New Orleans, martie 2023, rezumat publicat \u00een JACC 2023), <\/em>\u0219i valori mai mari ale scorului FAI de inflama\u021bie pericoronarian\u0103 (<em>Matyas B et al, rezumat publicat \u00een Eur. Heart J. Cardiovasc. Imaging, 2023<\/em>). \u00a0Totodat\u0103, inflama\u021bia epicardic\u0103 \u00eenainte de abla\u021bie a fost asociat\u0103 cu un risc crescut de recidiv\u0103 a fibrila\u021biei atriale la 12 luni <em>(Bordi L et al, JCE 2023), <\/em>iar indicele de inflama\u021bie epicardic\u0103 FAI s-a corelat semnificativ cu markerii de vulnerabilitate CT (<em>Parajko et al, rezumat publicat \u00een Eur. Heart J. Cardiovasc. Imaging, 2023<\/em>). De asemenea, pacien\u021bii cu un scor de calciu ridicat au prezentat diferen\u021be regionale \u00een distribu\u021bia inflama\u021biei coronariene, (<em>Rosca A et al, rezumat publicat \u00een Eur. Heart J. Cardiovasc. Imaging, 2023<\/em>), iar pl\u0103cile cu risc ridicat s-au constatat chiar \u0219i la pacien\u021bii cu un scor de calciu zero (<em>Matyas B et al, JCE 2023)<\/em>. Studiul de biomarkeri a ar\u0103tat c\u0103 I-CAM \u0219i MMP-9 sunt predictorii cei mai puternici pentru evenimentele adverse majore legate de vulnerabilizarea unei pl\u0103ci coronariene.<\/p><p>\u00cen paralel, activit\u0103\u021bile de diseminare au constat \u00een redactarea a 12 publica\u021bii in extenso (6 publicate \u00een reviste ISI cu factor de impact iar 6 under review), 7 comunic\u0103ri la congrese interna\u021bionale indexate ISI, precum \u0219i organizarea de evenimente de diseminare cu ocazia conferin\u021belor organizate sau sus\u021binute de membrii echipei de proiect, at\u00e2t online c\u00e2t \u0219i fizic.<strong> <br \/><\/strong><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-efc0656 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"efc0656\" data-element_type=\"section\" data-e-type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-9a23289\" data-id=\"9a23289\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-dc4b7d5 elementor-widget-divider--view-line_icon elementor-view-default elementor-widget-divider--element-align-center elementor-widget elementor-widget-divider\" data-id=\"dc4b7d5\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"divider.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-divider\">\n\t\t\t<span class=\"elementor-divider-separator\">\n\t\t\t\t\t\t\t<div class=\"elementor-icon elementor-divider__element\">\n\t\t\t\t\t<i aria-hidden=\"true\" class=\"fas fa-university\"><\/i><\/div>\n\t\t\t\t\t\t<\/span>\n\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-d3cfa7e elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"d3cfa7e\" data-element_type=\"section\" data-e-type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-49f8ae0\" data-id=\"49f8ae0\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-be899fe elementor-widget elementor-widget-text-editor\" data-id=\"be899fe\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong>Epicardial Fat, local Inflammation and Atheromatous Plaque Vulnerabilisation \u2013 a Computational Imaging Study based on Artificial Intelligence \u2013 IntelFAT<\/strong><\/p><p><strong><em>UEFISCDI PCE 206\/2021<\/em><\/strong><\/p><h2><strong>EXECUTIVE SUMMARY<\/strong><\/h2><p>The main objective of the project was to study the role of epicardial fat-mediated inflammation in the complex process of atheromatous plaque vulnerabilisation, using an innovative solution for quantification of the local inflammation at the site of the epicardial fat surrounding coronary arteries, based on advanced techniques of CCTA, artificial intelligence and cloud computing.<\/p><p>For achievement of this objective, the following activities were performing during the execution of the project:<\/p><p>-extraction of image data from the image bank available for the project.<\/p><p>-development of a new algorithm for selection and extraction of imaging characteristics using AI<\/p><p>-development of AI algorithms for assessment of inflammation at the level of epicardial adipose tissue<\/p><p>-biomarker study to correlate the CT characteristics of plaque vulnerability in 158 patients with high inflammation in the epicardial fat<\/p><p>-algorithm testing and validation for risk prediction, to assess their power to detect the risk of an acute coronary syndrome.<\/p><p>In total, the novel technology for detection of epicardial fat inflammation was tested in a number of 760 coronary arteries of 261 subjects, whose images were extracted from the image bank and further processed. The results indicated \u00a0a relevant role of epicardial fat inflammation in the complex process of coronary atheromatous plaque vulnerabilisation.\u00a0 We demonstrated that a high inflammation at this level may identify the site of a future plaque rupture\u00a0 <em>(Benedek T et al, EuroPCR, Paris, may 2023). <\/em>In post-COVID we found a significantly higher vulnerability associated with an increased inflammation at the level of the right coronary artery <em>(Benedek T et al, ACC congress, New Orleans, march 2023, abstract published in JACC 2023), <\/em>and increased levels of the fat attenuation index (FAI) score reflecting pericoronary inflammation (<em>Matyas B et al, abstract published in European Heart J. Cardiovasc. Imaging, 2023<\/em>). \u00a0At the same time, epicardial fat inflammation was associated with an increased risk of 12-months recurrence following atrial fibrillation ablation <em>(Bordi L et al, JCE 2023), <\/em>and FAI was significantly associated with vulnerability markers at CT (<em>Parajko et al, abstract published in Eur. Heart J. Cardiovasc. Imaging, 2023<\/em>). Moreover, subjects with increased coronary calcium score presented ragional variation in the distribution of pericoronary inflammation \u00a0(<em>Rosca A et al, abstract published in Eur. Heart J. Cardiovasc. Imaging, 2023<\/em>), and high-risk coronary plaques weer noticed even in patients with zero calcium score but increased inflammation (<em>Matyas B et al, JCE 2023)<\/em>.
